Purchase NDIS Consumables for Plan Managed Participants

If a plan manager is funded in your NDIS Plan, we can arrange to bill the plan manager once we receive a completed Plan Managed Participant Funding Authorisation Form from the Plan Management Agency.

Please note: If the Expected Spend per month not specified, a $1000 limit to purchases will be applied. Any higher limit will be at ADACSS Australian Disability and Aged Care Shop’s discretion and may be revoked if prompt payment does not occur.

If large, ad hoc orders are required, pre-payment can be arranged on a Proforma Invoice.

By completing and submitting this form, as Self Manging your NDIS Fund, you:

  1. acknowledges that you are responsible for ensuring that you are aware of what products can be purchased using their NDIS funding in line with their current plan and budget.
  2. acknowledges that you are responsible for paying all unpaid invoices that you purchased under their NDIS funding, including those from expired plans
  3. is solely responsible for advising ADACSS Australian Disability and Aged Care Shop in writing if your funding is materially reduced, ceases, or is altered.
  4. has obtained the authority of their client to use and share the information to facilitate the fulfilment of orders.

How to Order NDIS Consumables:

  1. Browse Our Online Store: Discover a wide range of products to support your independence.
  2. Add Items to Your Cart: Select the items you need and add them to your cart.
  3. Request a Quote: Send an email to sydney@adacss.com.au with your order details, including:
    • NDIS Agency-Managed Participants:
      • Your NDIS agency will process your order directly.
      • Please ensure that ADACSS Shop is registered as a provider with your agency.
    • Plan - Managed Participants:
      • Send an email to sydney@adacss.com.au with your order details, including:
        • Participant Details: Name, Date of Birth, and Contact Information.
        • NDIS Plan Details: Plan Start and End Dates.
        • NDIS Support Team (Optional): Plan Manager, Individual Planner, or Support Coordinator details.
