ADACSS Service Agreement for NDIA Managed Participants

To ensure we can provide the best service, all NDIA Managed Participants are required to complete the ADACSS Service Agreement and submit it to us for processing.

How to Submit:

  1. Download Managed NDIA Authorization Form here.
  2. Once you've completed the form, please email it to: sydney@adacss.com.au

How to Order NDIS Consumables:

  2. Add Items to Your Cart: Select the items you need and add them to your cart.
  3. Request a Quote: Click "View Cart" and then "Create Quote From Cart" to generate a quote.
  4. Review and Approve: Review your quote and confirm your approval.
  5. Complete Your Order:
    • NDIS Agency-Managed Participants:
      • Your NDIS agency will process your order directly.
      • Please ensure that ADACSS Shop is registered as a provider with your agency.
    • Self-Managed Participants:
      • Send an email to sydney@adacss.com.au with your order details, including:
        • Participant Details: Name, Date of Birth, and Contact Information.
        • NDIS Plan Details: Plan Start and End Dates.
        • NDIS Support Team (Optional): Plan Manager, Individual Planner, or Support Coordinator details.

Pay Out-of-Pocket for Faster Processing:

For quicker delivery, consider paying out-of-pocket and claiming reimbursement from your NDIS plan later.
